Direct visible buttons for Escalate and Photos in Survey Control

Current behavior is to click on the three dots, select raise/new related form, and select type. That's a lot of clicks. Escalate is a special related form that should be easily available in one click.

Even for photos, sometimes users don't even know that's an option.

The overall goal is to reduce the number of clicks, especially in the field app.
